We know you're busy, and getting to our office 15 minutes early to fill out forms can be inconvenient and difficult, so we'd like to help

We offer a few ways to make our new patient registration quick and easy. You can:

1. Fill out a few forms online and print out typed copies of all your forms (5-10 minutes)
2. Print out copies of blank forms and fill them in by hand at home (15 minutes)
3. Get a checklist of items to bring to our office 15 minutes before your first appointment (15-20 minutes)

If you choose option 1, none of your information is stored on our servers on a permanent basis. All of your data will be protected by SSL secure encryption, and none of the information you provide on our Web site is stored in a permanent database. It's simply used to provide you with pre-filled printable forms that can be brought to our office for use.
